Outreach

The Outreach Team will visit people in their own homes and will support individuals to be as independent, happy and fulfilled as possible. We also work with couples and families 18 years and over.

Our Outreach Team recognises that people are best supported to live well at home and support people to both maintain and develop daily life skills, while supporting people to engage in new opportunities i.e. employment, social and leisure.

The number of outreach hours provided can range from a few hours each week to daily visits.

Our outreach support is based on the needs of each individual. We work with each person to design and deliver what they need and want.

We support people with:

  • Learning Disabilities
  • Physical Disabilities
  • Dementia
  • Acquired brain injury
  • Mental health needs
  • Autism
  • Substance abuse
