Transition and Response Manager (12 Month Fixed Term)

The Role

This critical role is vital to Future Directions vision and values. As the Transition and Response Manager you will ensure a consistently high standard of care is delivered to everyone supported by the organisation when new services or people we support are in a period of transition. In this varied role, you will act as the project manager in the set up and establishment of services, whether they be new or have been transferred in from another povider.

The ideal candidate will be flexible so we can continue to support our staff and managers to deliver outstanding support to people around the clock. They will have had previous experience working with individuals with complex support needs as well as having previous experience of managing/supervising staff teams. Providing the best possible support for our people is a priority for us at Future Directions CIC, and in order to do that we continually improve our service.

Main Duties and Responsibilities

  • Project manage the set up and establishment of services, whether they are new services or have transferred in from another provider.
  • Effectively transfer responsibilities to a Team Manager once services have been established.
  • Work with Health & Social Services Departments and establishing/developing effective links with local agencies
  • Provide regular updates to senior managers about progress.
  • Develop and implement a project plan to set up the service within the agreed timeframes
  • Arrange ‘getting to know you’ process with the person(s), families and other relevant individuals
  • Participate in the staffing requirements and the recruitment and selection of the staff team
  • Manage the project plan and report progress to senior managers, escalating any major issues.
  • Ensure meaningful person centred plans are in place for each person supported.
  • Using expertise and knowledge to ensure that the health & support needs of people supported are identified and met through their support plans and that prescribed treatments are carried out
